Up to $100,000.00* in Life Insurance Coverage for Members

All active members, association members and life members of the Commissioned Officers Association of the United States Public Health Service who are in good standing, and who:

  1. Are under age 65; and
  2. Live in the United States; and
  3. Are officers on active duty; or
  4. Are officers in the inactive reserve; or
  5. Are former Commissioned Officers

Are eligible for this valuable term life coverage up to $100,000.00. *At age 65, or if you are already age 65, benefits reduce by 50%.

Your Spouse May Apply

Your spouse may apply for up to 50% of your coverage if he or she is under age 65, is not divorced from you, and resides in the United States. He or she will also be able to keep their coverage in the event you pass away. Spouse coverage terminates at age 70.

Pays a "Living Benefit" if You're Terminally Ill

If you or your spouse is Terminally Ill — having a life expectancy of 12 months or less — and you are covered for at least $10,000, you may request that a portion of the death benefit be paid early. This is the accelerated death benefit. The minimum benefit is $5,000 and the maximum benefit is $100,000 not to exceed 50% of the covered person's benefit amount.

Rates are based on the attained age of the Covered Person and increase as you enter each new age category.

When Coverage Begins:

Coverage for you and your spouse will become effective on the first day of the month following the Administrator's receipt of your application, approval and first premium payment. If on the date that you are to become covered under the policy or covered for increased benefits under the policy, you are not Actively-at-Work, are unemployed, or unable to carry on all the normal and customary activities of a person of like age and sex and in good health, or are confined to a hospital, you will not be covered until the day you are Actively-at-Work, are able to carry on all normal and customary activities of a person of like age and sex in good health, or the day after you are no longer confined to a hospital.

When Coverage Ends:

As long as you pay your premiums when due, remain an active member of the Commissioned Officers Association of the United States Public Health Service, and the Master Policy remains in force, it's possible to keep this coverage until you reach age 70. Coverage for you will terminate on the premium due date coinciding with or next following your attainment of age 70, or when you cease to be an Eligible Person as defined in the policy.

Coverage for your spouse will terminate when:

  1. The Master Policy is cancelled;
  2. Your premiums are not paid when due; 
  3. The date your coverage terminates; or
  4. The premium due date coinciding with or next following the date he or she attains age 70 or is divorced from you.


Suicide is not covered during the first two years of coverage. Benefits paid for death caused by suicide while sane or insane (in Missouri, while sane) within the first two years of the effective date of insurance are limited to a refund of your premiums paid.


Spouse means your spouse who is under age 65, resides in the United States, and is not legally separated or divorced from you.
